[PrintOwners] Straight Bill of Lading

I am looking for a supplier of a standard???? Straight Bill of Lading form. I have tried every business form company I can think of. Ennis, Deluxe, Western Business, even Office Depot, Office Max. They have them but not the sample that was provided to me. Can anyone suggest others? It is 8 1/2 x 11 with a laser snap on top. 3 part, has carbons, all copies white.
